Stack Slicer window titles must be distinct

Description
One major reason to use the Stack Slicer plugin is to create multiple windows that can be manipulated with other ImageJ plugins, such as the RGB Merge capability. Unfortunately, these plugins only behave properly on windows with distinct titles, and the Stack Slicer does not name each resultant window differently. The plugin should append relevant information to each window -- e.g., when splitting channels, append " - Ch 1", " - Ch 2", etc., to the titles.

Fixed, r3353; appropriate Z/C/T coordinates are appended to the original window title.
